An Advance Directive is a document that tells your health care provider and your family what type of care you would like if you are not able to speak for yourself. Advance Directives are appropriate for anyone, at any age, and can help you communicate your healthcare wishes if at any time you were incapacitated.
Community Hospital-Fairfax staff will be available to assist you, privately, in learning about Advance Directives and how to complete one. You do not need to RSVP, just stop by and have an important conversation about Advance Directives!
