• Priority Items: PHF’s (cheese, lettuce, and pico) not held at 41ºF or below (57ºF, 59ºF, and 44ºF) in prep table with missing bottom refrigerator door. Discussed putting minimal amounts out and discarding after four hours – temporary until door is replaced or blocked off to maintain temperature. No detergent or sanitizer (buckets empty) in ware washing machine (must use manual 3-bay sink until chemicals are resupplied!). Unlabeled spray bottle by 3-bay sink.

• Core Items: Food (onions, canned tomatoes) and food contact items (to-go trays) stored on floor. Prep table not maintaining temperature – bottom refrigerator door missing! Cardboard and aluminum foil lining shelving. No screens on vent hoods. Bad door gasket on True 2-door freezer in backroom. One dumpster lid open. Unshielded light bulbs. Grease floor vent hood dripping on outside sidewalk.

• Core Items: Some mold/mildew on coils in ice machine – not touching water or ice yet. A couple cracked light covers in kitchen ceiling fan. Discussed lighting – several light fixtures are not working – still have sufficient lighting so not a violation, but wouldn’t want many more to quit working.

• Priority Items: Hot dogs and tornados on roller grill not cooked or held at proper temperatures – cook to 165ºF and hold at 135ºF. Back/cooking portion of grill off – front only set at 124ºF! Discard and set temp higher/monitor with proper thermometer (COS). Wild morel mushrooms in kitchen – discussed (personal use given to Alex by customer). These are an unapproved food item and cannot be sold in the store. Pop ice not labeled for individual sale in Blue Bunny freezer. Ice/condensate from cooling unit in walkin freezer dripping on product (cookie dough and chunked chicken). Remove items and dispose of – do not store anything on these shelves until repaired/refrigeration repairman called – COS. Note: Kitchen staff called in – morning staff left kitchen well cleaned. Alex monitoring hot holding – all was pulled except 3 items of biscuits and gravy and roller grill/no lunchtime foods made due to staffing.

• Core Items: No handles on pizza prep table lids. Broken light covers above 3-bay sink. Formica top loose/taped on beverage counter top and restrooms. Women’s trash cans overflowing (COS). Door missing to men’s restroom. Doors to outside propped open with case of bottled water. Freezer cooling unit drain plugged – dripping inside onto product. Wall in disrepair under and around 3-bay sink and by mop sink. Broken tiles and missing mop board in restrooms. Broken/missing light covers in restrooms.
